New Patient Application
In this step, you will have the opportunity to review and agree to our patient policies. Once we have decided to embark on the journey to healing, we will send you a link to our patient portal.
In the patient portal, you will create an account and complete health questionnaire forms so that we can learn more about you and what services you will need. After you complete these questionnaires, we will schedule your first appointment.
This visit, which may last 2 to 3 hours, is our opportunity to get to know each other. This will begin our collaborative relationship.
We will review the intake questionnaires and any previous medical records or lab results that we have received. We will discuss with you your goals and determine diagnostic testing and treatment options as detailed below. We will start neurotransmitter precursor therapy trialing with amino acids. We will refer you to psychotherapy services if needed. We will focus on giving you the tools to start your journey towards brain health and a lasting recovery.
In light of COVID-19, all visits will be conducted over telehealth.

Our goal is to provide you with the highest level of patient centered care. We ask for a long term commitment from our patients, as time is required to see changes in brain chemistry, aid in relapse prevention, and sustain a brain-healthy lifestyle. Our approach includes functional medicine testing of your digestive tract, evaluating for metabolic, hormonal, and mineral imbalances and creating a comprehensive plan to address brain and body dysfunction.
For enrolling in our practice, you will receive the following services:
3 hours with MD for follow-up appointments. These visits will focus on review of lab results and your response to dietary, medication and nutrient supplementations.
Collaboration with your other practitioners, psychologists, therapists etc.
Up to 8 hours with health coaches/educators
10% discount physician grade supplements
Direct access to physician via online portal
Additional Levels of Service for Symptom and Withdrawal Management:
Many patients may require an additional level of service depending on the level of complexity of their use disorder and until symptoms and controlled-medication use have stabilized. These costs are in addition to the 6 month practice fee.
This plan is for patients who are actively detoxing from alcohol or drugs and is in need of daily check-ins, per American Society of Addiction Medicine guidelines. This is a highly intensive program.

Check ins: These are 15 minute telehealth appointments with Dr. Madhava to ensure that we are safely managing your symptoms
These levels of service will be determined by Dr. Madhava at the initial appointment
Patients may shift from one level of service to another as the nature of their addiction, symptomatology, and medication requirements change. Dr. Madhava will determine what level of service patients require.
Check-ins are not for laboratory reviews. Laboratory evaluation analysis will only be done during follow-up appointments.
Additional follow-up appointments past the included 4-hours will be billed at $650/hour. Payment is due before every visit.
Voicemail messages will be returned as quickly as possible, usually within a few hours
E-mails and direct portal messages will be returned as quickly as possible and within 24 hours
In unique circumstances, some patients may be eligible for a fee-for-service model. In this model, the following rates will apply:
$1000/hr for follow-up appointments
Any questions through the physician portal that cannot be answered in a yes/no format will require an appointment
Payment for all services is due upfront
